Please call my office at (216) 285-1799 to schedule an appointment. I schedule my appointments myself and I will review fees and office polices with you over the phone.

Please arrive 15 minutes early for your first appointment to fill out forms.

Payments

To ensure that you receive the highest quality of care and the strictest level of confidentiality, I accept fee for service payment only.
Payment is required at the time of visit.  Checks are preferred though credit card payment is an option as well.

I am out of network and not part of HMO or insurance company panels.

While the payment is due at time of service if you would like our office will submit your insurance claim to your insurance company and instruct them to send any reimbursement directly to you.
